Unemployment statements
SSA- 1099 if you were paid SS benefits
All 1099 forms
1099-MISC showing miscellaneous income
1099-R form if you are receiving a pension or annuity
1095-A from Health care provider
1099-C cancelation of debt on Credit Cards
All forms indicating federal income tax paid
Dependent care provider information.. name, employer ID, or SS number for
day care provider
All receipts or canceled checks if itemizing deductions
Social Security cards or other official documentation for you and all dependents
Picture ID
